Note: This policy does not apply to Integrated System Deployment and Support projects or applications maintained by Health System Computing. It does apply to all applications developed and maintained by Applications and Data Services.

|Reason for Policy:||Describes the general process for submitting an ITC/Applications and Data Services work request.|
|Definition of Terms in Statement:||Application & Data Services: A division within ITC that develops and maintains applications such as the Integrated Student Information System (ISIS), University Calendars, Course Offering Directory (COD), etc.|
All ITC/Applications and Data Services (ITC/ADS) systems development, enhancement and maintenance work is initiated by formal Work Requests submitted by users (this may also be in the form of an e-mail). No work can begin until the Work Request form has been completed and approved by appropriate user management, unless the request is a true emergency. All work request forms must be submitted to ITC/ADS.
Work request forms may be prepared by ITC/ADS, but the user remains responsible for ensuring the problem is accurately described. Requests must include a definition of problems to be overcome and objectives to be achieved. Desired results must be described as specifically and concisely as possible.
